Evergreen Squash Club Membership

Current Forms

Evergreen requires that all members have a hardcopy of the Liability Waiver on file at the club.

If applying for a full membership you will need to complete and submit a New Membership Application, and a Liability Waiver. In addition, if you wish to make your payment by credit card, or monthly electronic deductions, you will also have to complete an Electronic Payment Form.

If applying for a trial membership, you will need to complete and submit a Trial Membership Application, and a Liability Waiver.

Anyone wishing to contribute to Evergreen's Junior Development Program should complete and submit the Junior Program Development Donation Form

If applying for the Junior Training Program, at a minimum, you will need to submit a Fall 2012 Junior Training Program Application, and a Liability Waiver.

If the Junior is not already a member, a New Membership Application will also be required.
