Complete both of the following forms for each junior attending; and, submit (drop off or mail), with payment, to Evergreen Squash Club (1802 Glenaire Drive, North Vancouver, BC, V7P 1Y1).

1. 2012 Summer Camp Registration Form.
2. Liability Waiver.

The deadline for submitting registration, and payment, is Friday, August 10, 2012.
